Introduction:
The Aga Khan University Hospital in addition to its five Hospitals creates access through its Outreach Services which include Clinical Laboratories, Outpatient Medical Centres, Pharmacy services, Home Health and other services. Outreach Services are designed to provide primary to secondary healthcare services to patients near their homes and are present in several cities across Pakistan. Outpatient Medical Centres offer a range of specialty clinics including Family Medicine, Internal Medicine, Endocrinology, Dermatology, Gastroenterology, Cardiopulmonary, Orthopedics, Obstetrics and Gynecology, Pediatrics and Physiotherapy, and cater to children, adults, and elderly patients for general health checks or specific disease consultations.

Responsibilities:
You will be responsible to:
- Co-ordinates all processes for patient care and ensure timely delivery of all core processes.
- Manage scheduling and staffing of individual cohort.
- Conduct rounds at patients’ home, provide patient education and review documentation.
- Participates in multidisciplinary rounds as applicable and plan patient care accordingly.
- Acts as clinical resource person for staff and patients and conduct teaching sessions as needed.
- Provide remote assistance to staff working in home setting.
Requirements:
You should have:
- A Baccalaureate Degree in Nursing (BScN) from an accredited school
- Minimum three years of nursing experience
- Valid license from Pakistan Nursing Council
- Certification in Cardiopulmonary Resuscitation (CPR), Safe Medication Administration, Aseptic Techniques, I/V Cannulation
- In-depth knowledge of health and safety measures
- Good communication and interpersonal skills
- Willingness to work in shift duties
